1. Who we are
SLB Healthcare, LLC is a Georgia limited liability company doing business as SLB In-Home Care. We provide in-home skilled nursing and personal care support for medically fragile children in Georgia, including services under the Georgia Pediatric Program (GAPP). In this policy, “we,” “us,” and “our” mean SLB Healthcare, LLC.
This policy applies to this website, our contact and intake forms, our phone and text message communications, and the administrative records we keep about clients and prospective clients.
2. Information we collect
Information you give us directly
- Contact information: parent or guardian name, mailing address, email address, home phone, and mobile phone number.
- Child and family information: the child’s name, date of birth, county of residence, diagnoses, equipment and medication needs, and Medicaid status.
- Clinical information: physician orders, assessment findings, plans of care, nursing notes, and related medical records.
- Consent records: whether you consented to text messaging, the exact consent language shown to you, the date and time, and the method by which consent was given.
- Message content: the content of emails, text messages, and voicemails you send us.
Information collected automatically
- Basic web server and analytics data such as IP address, browser type, device type, pages viewed, and referring page.
Information from others
- Referrals and records from physicians, hospitals, discharge planners, Medicaid care management organizations, and state agencies, where permitted by law.
3. How we use your information
We use the information we collect to:
- Respond to your inquiry and determine whether your child may qualify for services
- Complete intake, enrollment, assessment, and plan-of-care documentation
- Schedule, staff, and deliver nursing and personal care services
- Communicate with you by phone, email, and — if you opted in — text message about appointments, scheduling, and care coordination
- Obtain authorization and payment from Georgia Medicaid, care management organizations, or other payers
- Comply with legal, licensing, accreditation, and regulatory obligations
- Maintain the security and proper functioning of our systems and website
We do not sell your personal information. We do not use your information for behavioral advertising, and we do not send marketing text messages.
4. Mobile information and SMS text messaging
Our commitment regarding mobile information
No mobile information will be sold, rented, or shared with third parties or affiliates for marketing or promotional purposes.
Text messaging originator opt-in data and consent will not be shared with any third parties. All other categories of information sharing described in this Privacy Policy expressly exclude text messaging originator opt-in data and consent.
The only exception is disclosure to vendors and subcontractors acting on our behalf strictly to operate the messaging service — for example, our SMS platform provider and the telecommunications carriers that transmit the message. These parties are contractually prohibited from using the information for any purpose other than delivering the service, and are prohibited from selling or sharing it.
If you provide a mobile number and check the SMS consent box on our contact page or intake documents, or otherwise opt in as described in our SMS Terms & Conditions, we will use that number to send you appointment reminders, scheduling and nurse-arrival updates, intake and eligibility follow-up, and care coordination messages.
Message frequency varies, typically 2–8 messages per month. Message and data rates may apply. You may reply STOP at any time to opt out, or HELP for help. Consent to receive text messages is not a condition of receiving care or of any purchase.
We keep a record of your consent — including the mobile number, the date and time, the exact language you agreed to, and the method of opt-in — for as long as you remain opted in and for a reasonable period afterward, in order to comply with the Telephone Consumer Protection Act (TCPA) and carrier requirements.
Standard SMS is not encrypted. See section 10 of our SMS Terms for how we limit protected health information in text messages.
5. When we share information
We share personal information only in the following circumstances, and always subject to the mobile information exclusion in section 4 above:

We never sell personal information. We never share mobile phone numbers or SMS opt-in consent with third parties or affiliates for their own marketing or promotional purposes.
6. HIPAA and protected health information
SLB Healthcare, LLC is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Health information we create or receive about your child is protected health information (PHI) and is governed by HIPAA, by Georgia law, and by our Notice of Privacy Practices, which is provided to every client at admission and is available on request.
Under HIPAA you generally have the right to inspect and obtain a copy of your child’s records, to request corrections, to request an accounting of disclosures, to request restrictions on certain uses and disclosures, to request confidential communications by an alternative means or at an alternative location, and to receive a paper copy of our Notice of Privacy Practices.
To exercise these rights, or to file a privacy complaint, contact us using the details in section 13. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
7. How we protect information
We use administrative, physical, and technical safeguards appropriate to the sensitivity of the information, including access controls limiting staff access to what they need for their role, encryption of data in transit on this website, secured electronic records systems, workforce privacy and security training, and business associate agreements with vendors that handle PHI.
No method of transmission or storage is completely secure. In particular, standard email and standard SMS are not encrypted, and you should not use them to send sensitive clinical details you would not want a third party to see.
8. How long we keep information
We retain clinical and billing records for the periods required by Georgia law, Medicaid program rules, and HIPAA — generally at least ten years, and for minors, generally through a period following the child’s twenty-first birthday. Inquiry and contact form submissions from people who do not become clients are retained for a shorter period. SMS consent records are retained while you are opted in and for a reasonable period afterward for legal compliance.
9. Your choices and rights
- Stop text messages. Reply STOP to any message, call (470) 570-7053, or email info@slbinhomecare.com.
- Stop emails. Use the unsubscribe link in any non-essential email, or contact us. We may still send you essential service and clinical communications.
- Access or correct your records. Submit a written request as described in our Notice of Privacy Practices.
- Alternative communication. Ask us to contact you only at a specific number, address, or by a specific method.
- Withdraw consent. You may withdraw consent for optional communications at any time without affecting your child’s care.
Georgia does not currently have a comprehensive consumer privacy statute. If you reside in a state that grants additional rights — such as the right to know, delete, correct, or opt out of sale or sharing — you may exercise those rights by contacting us; note that information governed by HIPAA is generally exempt from those statutes. We do not sell personal information, and we honor Global Privacy Control signals where applicable.
10. Cookies and website analytics
This website uses only the cookies and storage necessary for the site to function and, where enabled, basic aggregate analytics to understand how visitors use the site. We do not use advertising cookies, do not run retargeting pixels, and do not sell or share website data with advertising networks. You can block or delete cookies in your browser settings; the site will continue to work.
11. Children’s privacy
Our services are delivered to children, but this website is directed to parents, guardians, and healthcare professionals. We do not knowingly collect personal information online directly from children under 13. Information about a child is provided to us by a parent, guardian, authorized representative, or healthcare provider, and is handled as protected health information.
12. Changes to this policy
We may update this Privacy Policy from time to time. The current version and its effective date are always posted on this page. Material changes affecting how we use your information will be communicated to active clients. Your continued use of this website or our services after an update constitutes acceptance of the revised policy.
