By accessing or using this website, submitting a form, or communicating with us through the contact information provided here, you agree to these Terms of Use, our Privacy Policy, and — if you opt in to text messaging — our SMS Terms & Conditions. If you do not agree, please do not use this website.
2. No medical advice; no provider-patient relationship
The content on this website is general information about our services and about the Georgia Pediatric Program. It is not medical advice, nursing advice, legal advice, or a determination of eligibility for any benefit program. Using this website or submitting a form does not create a provider-patient relationship. A provider-patient relationship begins only after admission, a signed service agreement, physician orders, and, where applicable, payer authorization.
Emergencies: Do not use this website, our contact form, email, or text messaging to report a medical emergency. If your child is having a medical emergency, call 911 immediately.
3. Eligibility statements
Eligibility for the Georgia Pediatric Program and for Georgia Medicaid is determined by the State of Georgia and its designated agents, not by SLB Healthcare, LLC. Any statement we make about likely eligibility is an informed opinion, not a guarantee of coverage, approval, or a specific number of authorized hours.

When you submit a form or contact us, you consent to receive responses from us by the methods you provided — phone, email, or, if you separately and affirmatively opted in, text message. Email and standard text messaging are not secure or encrypted. Please do not send detailed clinical information, insurance identifiers, or other sensitive data by email or text unless we have arranged a secure method with you.
5. Acceptable use
You agree not to use this website to submit false or misleading information, to submit a mobile number you are not authorized to provide, to attempt to gain unauthorized access to our systems or data, to introduce malicious code, to scrape or harvest content or contact information, or to use the site in any way that violates applicable law or infringes the rights of others.

These Terms of Use are governed by the laws of the State of Georgia, without regard to conflict-of-law principles.
